Job summary

Justice Resource Institute in Taunton, MA is seeking an Intensive Care Coordinator to support youth under 21 with serious emotional disturbances. You will help coordinate care planning teams, link families to services, and advocate within child-serving systems to keep youth safely in the community.

Qualifications

• Bachelor’s or Master’s degree in social work, psychology, or related field required.
• Daily access to a reliable, insured vehicle and active Driver’s License.
• Flexible work schedule including evenings and some weekends.
• Experience navigating child/family-serving systems with state agencies and related entities preferred.
• Knowledge of Wraparound systems of care and child welfare is preferred.
• Background and driving record checks will be performed.

Responsibilities

• Facilitate Care Planning Teams; coordinate multiple services for MassHealth-enrolled youth under 21 with serious emotional/behavioral health needs.
• Act as liaison with individuals, partnering agencies, and supportive programs to guide youth care.
• Identify and assist families in accessing and monitoring available services and natural supports.
• Provide trauma-informed wrap-around services centered on youth needs.
• Support parents and caregivers in learning effective advocacy skills.
