Job summary

Health Care for the Homeless is seeking a Substance Use Counselor to provide outpatient SUD treatment and recovery support for youth and adults experiencing homelessness. You will use a trauma-informed, client-centered approach across fixed-site clinics, mobile clinics, and community settings to meet clients where they are and reduce barriers to care.

Qualifications

  • Associates degree required; Bachelor's degree preferred.
  • Licensed or certified by the Maryland Board of Professional Counselors (CSC-AD, CAC-AD, or LCADC) required.
  • Minimum two years of experience with individuals with substance use disorders and experience serving homeless populations preferred.

Responsibilities

  • Provide substance use and behavioral health assessments and interventions with harm-reduction, trauma-informed approach.
  • Engage and re-engage clients across clinic, mobile, and community settings to maintain treatment connection.
  • Provide individual and group counseling to support goals related to safer substance use and recovery.
  • Screening, intake, assessment, and referrals to levels of care including detox and outpatient services.
  • Lead or participate in groups to engage clients and support movement along the recovery continuum.
  • Collaborate with integrated care teams to develop individualized treatment plans with SMART goals.
  • Coordinate with MAT/ MOUD teams and ensure documentation for reimbursement.
