Qualifications

Responsibilities

• Oversee care for individuals assigned to Levels of Care to ensure continuity of care.
• Perform case management and coordinate services with clients, families, and providers.
• Administer assessments, documentation, and referrals per TRR or HHSC requirements.
• Engage in crisis assessment and respond to emergencies as needed.
• Maintain transportation, outreach, and community-based visits as required.
