Qualifications

• Experience in Foster Care Case Management preferred.

Responsibilities

• Conducts education programs on site, in the community, or in a family’s home.
• Enroll clients and assess the emotional and social needs of families.
• Support parents to help their children start healthy by ongoing assessment and education.
• Provide referrals to outside agencies to complete family goals.
• Possibly perform home visitation duties using program models and contracts.
• Engage families using a strength-based approach and promote self-directed change.
