Signature Health Inc. is seeking a Counselor, Outpatient & Crisis Services to provide outpatient counseling, crisis intervention, and care coordination for adults and children.

The role requires licensure in Ohio and involves team-based, trauma-informed treatment planning across multiple settings, including telehealth when appropriate. Qualifications

• Bachelor’s degree required; Master’s preferred.
• Active Ohio licensure (LSW/LISW/LPC/LPCC/LIMFT or MFT).
• AHA BLS certification required.
• Experience with crisis intervention, risk assessments and safety planning.

Responsibilities

• Conduct individual, group, and family therapy for adults and children.
• Develop and implement person-centered treatment plans using evidence-based practices.
• Provide crisis assessments and crisis intervention in various settings.
• Coordinate follow-up care and referrals to community resources.
• Maintain documentation in accordance with applicable standards.
