Qualifications

Responsibilities

• Independently assess and provide mental health and/or substance use services, including individual, group, and family counseling.
• Complete assessments, treatment planning, and risk monitoring; maintain EHR documentation and discharge planning.
• Collaborate with clients, caregivers, physicians, and teams to ensure continuity of care.
