Job summary

San Diego Youth Services seeks a Case Manager for the Youth Homelessness Program in San Diego. This full-time, non-exempt role reports to the Program Manager and provides trauma-informed case management and housing support to Transition Aged Youth (18–24).

Responsibilities include independent living skills development, safety planning, data entry, and linkage to resources. Evening/weekend availability and HIPAA-compliant practice are required.

Qualifications

• Bachelor's degree in social work, psychology, sociology or related field.
• Bilingual English/Spanish preferred.
• Two years’ experience with at-risk youth (18-24) experiencing homelessness preferred.
• Knowledge of Coordinated Entry System or willingness to learn.
• Experience with housing navigation and voucher systems.
• Valid driver’s license and reliable transportation.
• Availability for weekends and evenings.
• Background check, TB test; CPR/First Aid certification.

Responsibilities

• Provide trauma-informed case management and housing navigation for youth.
• Link youth to resources and independent living skills.
• Maintain confidential records and HIPAA-compliant documentation.
• Collaborate with staff and community partners to coordinate services.
• Support youth in accessing housing, education and employment resources.
• Work evenings and weekends as required.
