Job Summary
Under the supervision of the VP of Operations, the Transportation Manager is responsible for the management and operation of all activities related to transport services, including Pace participant appointments, Pharmacy deliveries, as well as Savory Fare deliveries. Oversees the coordination of safe and reliable transportation to medical appointments, activities, and other outings for all PACE clients. Works closely with the Pharmacy department to ensure timely medication deliveries. Collaborates with Savory Fare to deliver food weekly to customers. This position requires a high degree of problem-solving skills, organizational skills, and thorough and clear communication skills.
Job D Uties/Responsibilities
- Oversees the appropriate coordination of route logistics for the PACE driver fleet across all sites, alternative care settings, and medical appointments.
- Oversees and is responsible for the scheduling and routine delivery of participant medications; reports any related concerns to pharmacy staff as needed.
- Oversees the weekly delivery of Savory Fare products, ensuring great customer service and clear communication; works closely with Savory Fare and Kitchen Operations on delivery schedule and other needs.
- Ensures clear communication with all drivers and transportation aides regarding duties, responsibilities, assignment of runs, care of clients, unique situations, or delivery concerns.
- Ensures clear communication with all transportation staff and vendors to provide quality of care and safe transport.
- Works closely with Dispatcher and Dispatch/Driver to adjust schedules and runs based on unanticipated changes and determine the safest, most efficient routes.
- Ensures the timely transport of participants to and from the PACE center for clinic and outside medical appointments.
- Collaborates frequently with the Health Center to monitor scheduling efficiency or concerns.
- Collaborates with the intake department to coordinate transportation for potential participants.
- Serves as liaison to interdisciplinary team meetings for transportation staff, sharing essential client care and safety information.
- Participates in interdisciplinary team meetings to communicate relevant safety and environmental issues.
- Participates in the development of Care Plans for transport-related events and needs, setting measurable goals and strategies.
- Ensures compliance with routine vehicle maintenance schedules and communicates with vendors on maintenance, vehicle purchases, and supplies.
- Oversees ordering, requesting, delivery, and tracking of transportation department equipment and resources.
- Manages transportation budget under the supervision of the VP of Operations and plans for fleet growth.
- Ensures compliance with records/logs of passengers, mileage, service records, fuel purchases, transportation request forms, and other data.
- Monitors and analyzes PACE performance trends in utilization, financials, and participant outcomes, using data for decision-making.
- Completes vehicle spot checks to monitor adherence to policies and safe transport practices; monitors pick-up and drop-off at locations and observes driving practices.
- Ensures compliance with scheduling protocols and procedures.
- Assists with development, oversight, implementation, and adherence to transportation regulations, policies, procedures, and standards.
- Assists in developing staffing patterns and job descriptions for transportation staff and supports hiring and orientation.
- Addresses client concerns related to transportation promptly; assists with grievances and ensures proper documentation and resolution.
- Supports and implements new practices to improve customer experience with transportation and deliveries.
- Maintains knowledge of State and Federal regulations regarding transportation and reportable incidents; completes reports for compliance.
- Maintains professional relationships with vendors and represents PACE in meetings with the State Transportation vendor.
- Has complete knowledge and use of transportation and scheduling systems.
- Supports Day Center Directors by providing transportation services and collaborates with them to solve problems proactively.
- Coordinates with VP of Operations, Day Center Directors, and IDT to prepare emergency arrangements and ADC closures.
- Protects client confidentiality during scheduling, transport, and communications.
- Serves on organizational committees and attends meetings as requested.
- Participates in Quality Improvement studies and departmental quality initiatives.
- Participates in the PACE management team and attends management meetings.
- Communicates verbally and in writing with participants, families, providers, and the public about routine PACE operations; demonstrates strong writing and communication skills.
- Attends staff meetings, in-services, projects, and committees as assigned.
- Creates and implements departmental audit systems to identify non-compliance and quality improvement opportunities; uses data to guide decisions.
- Demonstrates the importance of clear documentation; completes accurate participant documentation in the electronic health record on time.
- Collaborates with and supports Canopy Care Solutions initiatives, integrating them as appropriate.
- Performs other related duties as required and assigned.

Physical Requirements:
Must be able to tolerate prolonged periods sitting at a desk and working on a computer.
Must be able to lift up to 10 pounds and at times up to 20 pounds.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.

3 to 5 years of relevant experience (logistics, transportation, program management or similar), Required
1 To 3 Years Of Staff Management Experience, Required
1-3 years of experience within a health care organization, Preferred
1 year of experience working with a frail or elderly population, Preferred

Driver's License & access to reliable transportation: community-based travel required.
