Job summary

Family Health West in Fruita, CO seeks a Transportation Driver to support patient transport to medical appointments and services. You will become part of a team focused on safe, timely, and courteous transport within the facility network.

The role requires a Colorado driver’s license (CDL preferred but not required) and about one year of related experience or training. Shifts include Thursdays and Fridays, with on-call coverage as needed.

ESSENTAIL FUNCTIONS:

• Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
• Communicate necessary information to others as appropriate.
• Transport patients and residents to medical appointments, services and special trips as scheduled through the Transportation Department. Be attentive to the daily driver schedule.
• Assist with loading and unloading passengers.
• Assist in coordinating patient transport activities and services with other departments as requested.
• Function as a liaison between the Transportation Department and other departments.
• Interact as necessary with nursing and other support departments to ensure the quality and timely delivery of service to patients/residents, physicians, visitors and staff.
• Ensure the proper operation of transportation related software programs and systems.
• Maintain a safe, healthy and orderly work environment to ensure dispatch equipment is in good, clean working condition.
• Recommend and participate in the solutions of on-going problems or trends regarding patient/resident transport.
• Develop and promote positive effective customer service relations with patients/residents, family members, visitors and staff.
• Keep the Transportation Manager and/or Fleet Supervisor informed of activities, needs and/or concerns.
• Participate with assigned transportation staff with on-call coverage as needed.
• Assist with after-hours non-emergency transportation services as requested.
• Follow proper communication procedures to ensure driver, resident and/or passenger safety.
• Maintain all the necessary driver, insurance and CDL license requirements as directed by Family Health West.
• Complete all necessary pre/post trip vehicle safety inspections.
• Assist with routine interior and exterior cleaning/washing of all Family Health West owned vehicles.
• Other job-related duties as assigned.

EDUCATION:

Education: High School Diploma or equivalent preferred.
Licenses: Current Colorado Driver’s License required.
CNA license preferred; not required.
Certifications: Proof of current personal automobile insurance.
Experience: One-year related experience and/or training preferred.

WAGE STARTS AT $18.00 per hour and goes up based on experience
Schedule: Thursday & Friday (7:30 am - 4:00 pm) 16 hours per week
Call required with a rotation of every 4 weeks
On-call for Holidays & weekends as needed
