Your Job

Responsible for coordinating transportation and transportation of residents to and from physician appointments in a timely manner and under the supervision of licensed nursing personnel.

Qualifications

• Valid Driver's Licensure for the state in which you are applying.

Responsibilities

• Organize transportation of residents to appointments based on priority of needs.
• Transport residents to inpatient and outpatient programs, facilities, and residences.
• Develop driving schedules based on resident appointments and needs.
• Report, assess, and communicate any changes in residents' conditions during transportation.
• Transport medical records for signatures.
• Maintain safety policies and vehicle condition.
• Assist clinical staff with resident mobility as needed.
• Establish positive communication with outside facilities and providers.
• Report scheduling changes and follow up as necessary.
• Provide hydration and protect residents' personal property.
• Perform other duties as assigned.
