Job summary

Emerald Nursing and Rehab Columbus seeks a Transportation CNA to join our team in providing compassionate, resident-centered care in a post-acute setting. As a Transportation Aide, you will assist with daily living activities and transport residents to appointments under supervision.

You will help maintain a dignified environment, ensure resident comfort, assist with meals, and support data collection for care plans and admissions.

Qualifications

• Must be a Certified Nurse Aide in good standing with the state.
• Must maintain a current CNA certification throughout employment.
• Must achieve 12 hours of in-service education annually as required by federal regulations (as well as state requirements).
• Must be able to maintain confidentiality regarding patient and company proprietary information.
• Must have the ability to relate professionally and positively and work cooperatively with residents, families and other employees at all levels.
• Must be capable of maintaining regular attendance.

Responsibilities

• Provide direct resident care under supervision of licensed nursing personnel.
• Transport residents to appointments as scheduled.
• Assist residents with activities of daily living including bathing, dressing, grooming, toileting, bed linens, and positioning.
• Prepare residents for meals and snacks; assist with eating and record intake.
• Maintain residents' rooms in neat, orderly condition and ensure correct diet.
• Support resident participation in activity programs.
• Respond to residents and family members' needs and concerns.
• Ensure residents' rights are honored at all times.
• Contribute data for accurate completion of the MDS and plan of care.

We are seeking a compassionate and dedicated Transportation Certified Nursing Assistant to join our team! As a Transportation Aide, you will provide direct resident care under the supervision of licensed nursing personnel and transport residents to appointments as scheduled. You will also assist with promoting a compassionate physical and psychosocial environment for the residents.

Responsibilities:

• Clear driving record
• Willingness to transport as needed
• Flexible hours
• Properly use and maintain facility equipment
• Assist residents with activities of daily living including bathing, dressing, grooming, toileting, changing of bed linens, and positioning in and out of bed, chair, etc.
• Prepare residents for meals and snacks
• Assist residents in eating where needed and record food intake
• Maintain residents' rooms in neat, orderly and clutter-free manner
• Ensure residents are given correct diet
• Support residents' participation in activity programs
• Directly respond to needs and concerns of residents and family members
• Ensure residents' rights are being honored at all times
• Provide resident care as directed by care plan
• Ensure residents' comfort while assisting them in achieving their highest practicable level of functioning
• Participate in collecting data needed for the accurate completion of the MDS and the plan of care
• Other responsibilities, within scope, as set forth by DON/ADON
