Job summary

Brookestone Meadows is seeking a dependable Transportation/CNA to transport residents safely to appointments and activities while providing daily living assistance. The role emphasizes dignity, respect, safety, and excellent service.

Responsibilities include helping residents board vehicles, performing safety checks, and assisting with daily care activities as assigned. Strong communication and teamwork are essential in delivering quality resident care.

Qualifications

• Current Certified Nursing Assistant (CNA) certification in good standing.
• Valid driver's license and an acceptable driving record.
• Ability to safely assist residents with transfers and mobility.
• Strong communication, customer service, and interpersonal skills.
• Dependable, organized, and able to work independently and as part of a team.
• Compassionate attitude and a genuine desire to serve older adults and individuals requiring skilled nursing care.

Responsibilities

• Safely transport residents to and from medical appointments, scheduled outings, and other approved destinations.
• Assist residents with entering and exiting vehicles, using appropriate lifts and mobility equipment as needed.
• Complete required vehicle safety checks and ensure the vehicle is clean, safe, and properly maintained.
• Maintain accurate transportation schedules, documentation, and trip records.
• Assist nursing staff with resident care and activities of daily living, including bathing, dressing, grooming, toileting, and mobility, as assigned.
• Monitor resident comfort and safety during transportation and while providing care.
• Communicate effectively with residents, families, staff, and medical offices.
• Follow infection prevention procedures, facility policies, and all applicable safety requirements.
• Protect resident privacy and maintain confidentiality.
• Promote a positive environment that reflects the mission, vision, and values of Brookestone Meadows.

In this dual-role position, you will help residents get safely to and from appointments and activities while also providing quality resident care and assistance with daily living needs. We are looking for someone who values dignity, respect, safety, and excellent customer service.
