Qualifications

Responsibilities

• Assesses, plans, implements, evaluates, and documents daily patient care.
• Administers medications and treatments per policy and physician orders.
• Provides patient/family education and uses Teach-Back to confirm understanding.
• Collaborates with inter-disciplinary team to coordinate care.
