Job summary

Qualifications

• Master's degree in Social Work is required.
• No experience required; three (3) years post-master's experience in healthcare preferred.
• Bilingual skills preferred.

Responsibilities

• Completes in-depth transplant evaluations to determine candidacy for transplant.
• Provides counselling and support to patients and families through the transplant process.
• Develops discharge plans with the multidisciplinary transplant team.
• Links patients to Medication Assistance Programs and community referrals as needed.
• Educates and supervises MSW students during internship.
• Performs policy development, problem solving, and staff training.
