Job summary

Northwestern Medicine in Chicago seeks a Transplant Nurse Coordinator - PNE. You will be a registered nurse coordinating care across stem cell or organ transplantation, practicing under transplant physicians.

Under supervision, you coordinate referrals, tests, follow-ups, and patient education while collaborating with the interdisciplinary team to optimize outcomes and cost-effective care.

Qualifications

• BSN required and IL RN license.
• CPR-BLS certification required.
• 2–3 years of related clinical experience.

Responsibilities

• Plan, direct, provide, and document care for transplant patients.
• Coordinate comprehensive care including consent, data collection, follow-ups, tests.
• Collaborate with interdisciplinary teams to maximize wellness and patient outcomes.
• Educate patients, families, and staff based on individual needs.
• Serve as liaison between patient, family, and external hospitals.
• Ensure practice aligns with policies, procedures, and regulatory standards.
