Overview

The Clinical Transplant Coordinator, in collaboration with physicians and other healthcare professionals, is responsible for managing the clinical care of patients and families throughout the liver transplantation process.

Responsibilities
  • This role also involves close collaboration with transplant surgery, hepatology, and referring providers to optimize care coordination for patients with malignancies such as colorectal cancer and hepatocellular carcinoma (HCC).
  • Key responsibilities include active participation in tumor boards, coordination of multidisciplinary treatment pathways, and contribution to the development of protocols for managing complex medical conditions associated with liver failure.
  • The coordinator applies broad clinical expertise while serving as a clinician, educator, collaborator, and patient advocate.
  • Functioning in a semi-autonomous, self-directed capacity, the coordinator provides both direct and indirect patient care in accordance with professional practice standards and evidence-based clinical guidelines.
Requirements
  • Bachelor's degree in Nursing required Current licensure and registration (or valid permit) as a registered professional nurse with the New York State.
  • A minimum of two years of clinical experience in a highly specialized complex care program.
  • Spanish speaking preferred.
  • Experience in transplant patient care role preferred.
  • Effective English communication skills, both written and oral.
  • Ability to use data entry and retrieval system.
