Join our team as a Transplant Care Nurse RN specializing in Stem Cell and Bone Marrow, working remotely. This role involves implementing effective utilization and case management strategies for an assigned member panel, assessing health management needs, developing care plans, and coordinating care to improve health outcomes. You will collaborate with providers to ensure quality and timely care delivery.

Responsibilities include:

• Overseeing a panel of members, assessing their health needs, and identifying appropriate clinical interventions.
• Creating and implementing care plans with both short and long-term goals.
• Documenting all activities in compliance with regulatory and business standards.
• Performing outreach to members, addressing barriers, and coordinating resources.

Qualifications:

Education: High School/GED required; Bachelor's in Nursing preferred.

Experience: At least 7 years in clinical, case/utilization management, or related fields; 1 year in a clinical setting; transplant and bone marrow experience preferred.

Licenses & Certifications: Current RN licensure; certifications in utilization management or case management preferred.
