Job summary

ACH Child and Family Services seeks a Transitions Program Manager to lead The LIFE Project and Families Together Program, supervise LIFE Project Mentors and the Case Manager, and ensure high-quality, compliant services across sites.

Requirements include a Bachelor's in Social Work; a Master’s in Social Work is preferred, with 3 years of supervisory experience. Will drive to project sites and coordinate with external partners.

Position Function

The Transitions Program Manager is responsible for the development, maintenance and evaluation of The LIFE Project and the Families Together Program, ensuring quality services are provided to the clients as well as compliance with best practice standards and grant and contract requirements. Responsibilities include supervising the LIFE Project Mentors, and the Families Together Case Manager, monitoring the day-to-day operation of The LIFE Project settings and securing The LIFE Project settings. The job requires sensitivity to the service population’s cultural and socioeconomic characteristics and knowledge of best practices for services, program development, and supervision.

Requirements

Education: A bachelor’s degree in Social Work or related field from an accredited college or university is required; A master’s degree in Social Work or related field is preferred. Clinical licensure (LCSW, LPC, LMFT) preferred.

Experience: Three years of supervisory experience and skill in social service programs with preference given to those who have worked with young adults aging out of foster care or with young adults in community programs. Two years of supervision experience required.

Working Conditions

The position requires the ability to drive to project sites. Exposure to clients with behavior problems, physical aggression and mental health issues like PTSD and depression. Possible exposure to illness and blood.

Key Expectations/Responsibilities

Supervision and Planning

• Hires, supervises and supports the Families Together Case Manager and The LIFE Project Mentors, and assists them with overall program implementation
• Administers and coordinates regular staff meetings, training, evaluation of programs, policy and procedure development and application
• Builds community linkages, awareness and promotion of the program
• Continually assesses the education needs of staff in the programs
• Assists the Director of Residential Services-Wichita Campus with budgeting and manages expenditures affecting each program
• Develops and maintain relationships with other ACH programs as well as external collaborators
• Evaluates and encourages continuous improvement and coordination of all aspects of each program

Implementation

• Ensures that programs meet all contract and/or grant requirements and meet or exceed outputs and outcome measures
• Coordinates the identification and negotiation of The LIFE Project settings
• Engages and works with landlords to maintain collaborative and cooperative relationships
• Monitors the Families Together Program and The LIFE Project shared housing sites for cleanliness to ensure a safe and clean environment
• Ensures The LIFE Project housing settings have all required furnishings and leasing requirements are satisfied
• Coordinates and communicates with CPS and OCOK on referrals of candidates for The LIFE Project
• Collaborates with the Transitions Clinical Manager as well as the Families Together Case Manager to deliver quality assessment services and coordinate intakes for prospective clients
• Reviews and evaluates documentation of client services and completes all documentation required for grants and contracts for the programs, including the completion of all Federal Home Loan related documents as well as weekly completion of the Supervised Independent Living vacancy report
• Ensures regular delivery of life skills training to clients enrolled in The LIFE Project and Families Together Program
• Maintains client records in accordance with policies, procedures and COA standards and ensure that they are securely stored
• Monitors for the implementation of Trauma-Informed Care and selected evidence-based practices used by program staff
• Coordinates Connections for Life activities with Volunteer Coordinator
• Documents and develops systems to track outcomes, outputs for program evaluation and feedback on the effectiveness of the programs
• Maintains regular communication with the Director of Residential Services-Wichita Campus regarding program status, critical incidents and on-call situations
• Notifies supervisor, appropriate authorities and/or DFPS contract manager of serious incidents, setting changes, pending discharges, and discharges from The LIFE Project within timelines prescribed by the contract
• Provides on-call support for clients on a rotating basis with the Transitions Clinical Manager and Families Together Case Manager
• Informs Director of Public Grants and Contracts of any plans to make material changes in program or program operations versus original grant or contract response and works with Grants and Contracts Team to submit request to DFPS in advance of making materials changes
