Elevance Health is seeking a Transitions of Care RN for a 100% virtual role. The position involves patient education, health coaching, and care coordination for members with chronic conditions. Ideal candidates will have an AS in nursing and relevant clinical experience. This role offers flexibility and a comprehensive benefits package.
This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.
Work shift: 4/10 work shift with rotating weekends and holidays per business needs.
The Transitions of Care - RN, 100% Virtual, CareBridge is responsible for delivering patient education, disease management interventions, and health coaching for members across multiple lines, focusing on health improvement and management of chronic diseases.
AS in nursing and at least 2 years of condition-specific clinical, home health, or discharge planning experience, or equivalent education and experience. Current unrestricted RN license in applicable state(s) required.
The salary range for candidates working in Colorado or Maryland is $68,880 to $108,240. Elevance Health offers a comprehensive benefits package, including incentives, stock purchase plans, 401(k), health benefits, and wellness programs. Candidates must reside within 50 miles or a 1-hour commute of an Elevance Health location and be vaccinated against COVID-19 and Influenza unless an exemption is provided.
