Transitions of Care RN- 100% Remote, CareBridge

The Transitions of Care RN- 100% Virtual role involves delivering patient education, disease management interventions, and health coaching for members with chronic diseases. The position offers full-time remote work with required in-person training sessions, working a 4/10 shift with rotating weekends and holidays.

Primary responsibilities include:

• Conducting behavioral or clinical assessments to identify member needs.
• Developing and coordinating health coaching plans.
• Collaborating with healthcare providers to implement care plans.
• Using motivational interviewing to promote health behavior change.
• Monitoring and adjusting interventions as needed.
• Referring members to appropriate facilities and managing complex cases.

Minimum requirements:

• Associate Degree in Nursing (ADN) and at least 2 years of clinical or discharge planning experience, or equivalent education and experience.
• Current unrestricted RN license in applicable state(s).

Preferred skills and experiences include:

• Bilingual abilities.
• Experience with adult and pediatric chronic illness, home health, utilization, or case management.
• Knowledge of electronic documentation systems.
• Excellent communication skills over the phone.
• Experience managing patients with multiple chronic conditions.
