Transitions of Care RN - 100% Remote, CareBridge

Join us as a Transitions of Care RN - 100% Remote, CareBridge at Elevance Health.

Position Overview

This is a fully virtual role, requiring associates to work remotely with in-person training sessions. The shift is 4/10 with rotating weekends and holidays. The RN will participate in patient education, disease management, and health coaching across multiple chronic disease programs.

Key Responsibilities
  • Conduct behavioral or clinical assessments to determine member needs.
  • Develop and coordinate personalized health coaching plans.
  • Collaborate with healthcare providers to implement care plans.
  • Educate members on clinical needs and facilitate referrals.
  • Utilize motivational interviewing techniques to promote health behavior change.
  • Monitor and evaluate intervention effectiveness, adjusting plans as necessary.
  • Refer members to appropriate facilities and services.
Minimum Requirements
  • Associate Degree in Nursing (ADN) or higher with at least 2 years of clinical or discharge planning experience; or equivalent education and experience.
  • Current unrestricted RN license in applicable state(s).
Preferred Skills and Experience
  • Bilingual abilities strongly preferred.
  • Current, unrestricted, Compact RN license preferred.
  • Experience with adult, chronically ill, pediatric, or IDD patients.
  • Home health, utilization management, or case management background preferred.
  • Previous care transition experience is a plus.
  • Proficiency with electronic documentation systems.
  • Excellent communication skills, especially over the phone.
  • Experience caring for patients with multiple chronic conditions such as COPD, CHF, CKD, etc.
