Job summary

MemorialCare in Fountain Valley, CA seeks a Transitions of Care Pharmacist Per Diem to support safe medication use on admission and discharge. Under supervision of the Clinical Pharmacy Program Manager, the TOC Pharmacist focuses on medication reconciliation, evidence-based management, and smooth transitions between inpatient and outpatient settings.

Requirements include an active CA Pharmacist license and at least 3 years of acute care clinical pharmacy experience; PGY1/TOC residency preferred.

Qualifications

• Active CA pharmacist license required.
• Minimum 3 years clinical pharmacy experience in an acute care setting.
• Direct experience with admission and/or discharge medication reconciliation preferred.
• PGY1 TOC residency or PGY2 TOC residency strongly preferred.
• Excellent communication and cross‑disciplinary collaboration skills.

Responsibilities

• Obtain best possible medication history for high‑risk patients at admission.
• Perform discharge medication reconciliation for high‑risk patients.
• Contribute to quality improvement initiatives in reconciliation workflows.
• Develop TOC workflows, guidelines, and training.
• Be reliable and punctual; follow policy and maintain professional conduct.
• Interact constructively with physicians, nurses, case management.
• Support systemwide efforts to reduce medication-related harm.
