Job summary

MemorialCare is seeking a Transitions of Care Pharmacist Per Diem in Fountain Valley, CA. Under the supervision of the Clinical Pharmacy Program Manager, the TOC Pharmacist focuses on accurate medication reconciliation and safe transitions between inpatient and outpatient settings.

Qualifications

• Active CA pharmacist license required.
• Minimum of 3 years of clinical pharmacy experience in an acute care setting.
• Direct experience performing admission and/or discharge medication reconciliation preferred.
• PGY1 Pharmacy Residency or PGY2 TOC Residency preferred.

Responsibilities

• Obtain the best possible medication history on admission and reconcile medications.
• Perform discharge medication reconciliation for high‑risk patients.
• Participate in quality improvement initiatives to improve reconciliation accuracy and workflow.
• Develop TOC workflows and educate staff involved in reconciliation and transitions of care.
