Job summary

CentraCare Health in Minnesota seeks a Transitions of Care & Anticoagulation Stewardship Pharmacist to lead safe medication management as patients move across care settings. You will build and optimize care plans with the care team, addressing barriers like affordability and prior authorizations while mentoring residents and students.

You will also serve as a clinical expert for anticoagulation, designing services, expanding programs, and driving patient outcomes with interdisciplinary teams.

Qualifications

• Bachelor of Science in Pharmacy or Pharm.D. required.
• Advanced training (residency/fellowship) and/or hospital experience preferred.
• Current Minnesota Board of Pharmacy licensure.

Responsibilities

• Lead transitions of care and anticoagulation stewardship activities.
• Partner with physicians, nurses, care coordinators, and pharmacists to design care plans.
• Serve as preceptor for PGY-1 and APPE rotations in pharmacy.

TheTransitions of Care & Anticoagulation Stewardship Pharmacistplays a vital role in ensuring safe, accurate, and seamless medication management as patients transition across healthcare settings, including hospital to home and between inpatient and outpatient care environments.

In thishighlycollaborative role, you will partner with physicians, nurses, care coordinators, community pharmacists, and other healthcare professionals to createmedication plans that support exceptional patient outcomes. You will identify and resolve potential discharge barriers, including medication affordability, access challenges, prior authorizations, and compounding needs, helping patients successfully navigate transitions of care.

As theAnticoagulation Stewardship Pharmacist, you will serve as a clinical expert andtrustedresource for patients receiving anticoagulation therapy. Working closely with interdisciplinary teams, you will develop, implement, and optimize care plans that balance thrombosis prevention with bleeding risk reduction. This role offers a unique opportunity to design, expand, and grow a developing anticoagulation stewardship service.You will build your own practice, drive program development, establish collaborative relationships, and create a lasting impact.

In addition to direct patient care responsibilities, this position serves as the primary preceptor for a CentraCare PGY-1 Pharmacy Resident rotationand anAPPE student rotation, providing the opportunity to mentor and develop the next generation of pharmacists.

Schedule:

• Full-time | 80 hours every two weeks
• Days | Monday-Friday | 8-hour shifts start between 7am-8am
• Occasional weekends and holidays

Pay and Benefits:

• Starting pay begins at $62.10 per hour and increases with experience
  • Pay range:$62.10-$93.15 per hour

Qualifications:

• Bachelor of Science in Pharmacy degree and/or Pharm. D.
• Advanced training (residency, fellowship, and/or board certification) and/or previous hospital experience preferred.
• Current licensure by the Minnesota Board of Pharmacy.
