• Develop and implement individualized transitional care plans across medical, social, and functional needs.
• Educate patients and caregivers on diagnosis, treatment, medications, and community resources.
• Advocate for patients, coordinate needed services, and address barriers to care.
• Collaborate with physicians, case managers, pharmacists, social workers, rehab staff, and community agencies to ensure safe transitions.
• Maintain accurate documentation in electronic medical records and dashboards.
• Participate in interdisciplinary meetings to align care goals across providers.
