Job summary

MUSC Community Physicians (MCP) is seeking a Transition Care Nurse (RN) to support safe patient transitions from hospital to primary care via telephonic outreach and timely follow-up.

You will review discharge information, assign cases by acuity, document calls, and communicate priorities to providers, helping prevent unnecessary acute care use and supporting value-based care in South Carolina.

Qualifications

• RN with 2+ years of clinical experience.
• Experience in community health education, case management or primary care.
• ADN required; BSN preferred.

Responsibilities

• Assist with data metrics to measure impact.
• Serve as patient resource for information, education and follow-up care coordination.
• Initiate documentation to complete transitional care appointments with the primary care provider.
• Reinforce discharge plan and follow-up importance with patients.
