Job summary

The University of Maryland Medical System is seeking an Advanced Practice Provider (APP) to enhance outpatient care for patients in the Transitional Care Center. The APP delivers patient-centered services aimed at improving care access and supporting safe transitions from hospital to home.

Key responsibilities include performing comprehensive evaluations, conducting patient assessments, managing acute conditions, and collaborating with healthcare teams. Ideal candidates are licensed Nurse Practitioners or Physician Assistants with previous primary care experience.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current and unrestricted state licensure as a Nurse Practitioner or Physician Assistant.
• Current national certification through an approved certifying body (ANCC, AANP, or NCCPA).
• Current Basic Life Support (BLS) certification required.
• Prior experience in primary care, hospital medicine, internal medicine, family medicine, or transitional care preferred.
• Bilingual candidates preferred.

Responsibilities

• Performing comprehensive post-hospitalization evaluations.
• Conducting patient assessments including medical history review.
• Managing acute and chronic medical conditions.
• Performing medication reconciliation and adjusting regimens.
• Ordering and interpreting laboratory tests.
• Identifying and managing complications post-hospitalization.
• Providing patient and family education.
• Facilitating safe care transitions with primary care providers.
• Assisting patients in establishing care with a primary care provider.
• Supporting initiatives to reduce hospital readmissions.
