Job summary

University of Chicago Medicine is seeking an Advanced Practice Provider (NP or PA) for the Care Transitions Clinic. The APP will deliver direct patient care, manage a caseload, and exercise autonomous decision-making under physician supervision.

The role includes in-person and tele-health visits, order entry, and coordination of care to reduce ED visits and readmissions. Collaboration with physicians is essential, with NP/PA certification and IL licensure required, plus 1 year of APN/PA

Qualifications

• Master’s of Science in Nursing.

• Must have or qualify for Illinois RN and APN licensure/registration.
• DEA prescribing number eligibility.
• At least 1 year of APN experience preferred.

Job Description

Join the University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) – Care Transitions Clinic.

The Advanced Practice Provider will be responsible for comprehensive direct patient care management and will manage a caseload of patients by exercising autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The APP will provide healthcare and patient services that may include education, research, and clinical support, as well as complete administrative activities under the direction and supervision of the Director/leadership. The APP has acquired the knowledge, skills, and judgment necessary to assist the physician through organized instruction and supervised practice.

The Care Transitions Clinic (CTC) APP will perform in-person and tele-health ambulatory care visits for a variety of patient populations with both acute and chronic needs and in-patient chronic disease hospital consults. A broad experience base of Primary Care, Urgent Care, Oncology and/or a variety of experience in several Medical Specialties will be helpful for the person in this role. All programs and services of the Care Transitions Clinic are aimed at reducing Emergency Department utilization, hospital admissions, and re-admissions.

Nurse Practitioner Education And Experience Requirements

• Master’s of Science in Nursing
• Current and/or eligibility for State of Illinois RN and APN licensure/registration
• Must be certified as a Nurse Practitioner by the ANCC or AANP
• Must have or be able to qualify for Illinois and DEA prescribing numbers
• At least 1 year of APN experience preferred

Physician Assistant Education And Experience Requirements

• Completion of an accredited (physician assistant) educational program
• Certification in ACLS and by the National Commission on the Certification of Physician Assistants (NCCPA) and current and/or eligibility for State of Illinois licensure/registration
• At least 1 year of experience preferred
