Join the University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) – Care Transitions Clinic.
The Advanced Practice Provider will be responsible for comprehensive direct patient care management and will manage a caseload of patients by exercising autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The APP will provide healthcare and patient services that may include education, research, and clinical support, as well as complete administrative activities under the direction and supervision of the Director/leadership. The APP has acquired the knowledge, skills, and judgment necessary to assist the physician through organized instruction and supervised practice.
The Care Transitions Clinic (CTC) APP will perform in-person and tele-health ambulatory care visits for a variety of patient populations with both acute and chronic needs and in-patient chronic disease hospital consults. A broad experience base of Primary Care, Urgent Care, Oncology and/or a variety of experience in several Medical Specialties will be helpful for the person in this role. All programs and services of the Care Transitions Clinic are aimed at reducing Emergency Department utilization, hospital admissions, and re-admissions.
