Transitional Care Management Nurse (LPN) - Remote Position

Position Details
  • Full Time (Monday-Friday 8AM-5PM)
  • On-Site Training - Remote/Jacksonville, FL

Millennium Physician Group is seeking to hire a full-time Transitional Care Management Nurse (LPN) to join the ACO team. We are looking for a positive, energetic, well-organized team member who can multi-task and think outside the box, supporting our goal of providing an excellent patient experience.

Training Schedule

In-Person Training for 2-3 weeks onsite, after which the role becomes fully remote.

Role Responsibilities
  1. Communicate with patients discharged from hospitals or Skilled Nursing Facilities within 24-48 hours.
  2. Coordinate with community services utilized by the patient.
  3. Identify and facilitate access to community resources and health services.
  4. Educate patients and families to support self-management and independent living.
  5. Document responses and chart in the patient's EMR.
  6. Schedule face-to-face visits with PCP.
  7. Support treatment adherence and medication management.
  8. Handle incoming calls, faxes, and perform office tasks as assigned.
Compensation and Benefits

Qualifications
  • Completion of an accredited LPN program and current Florida LPN license preferred.
  • Two years of related experience or equivalent education and training.
  • Current certifications/licenses such as LPN, CPR, etc.
