Diversicare is seeking a Clinical Liaison to support admissions in its care centers. You will perform clinical assessments, work with care teams to ensure smooth transitions, and clearly communicate Center benefits to patients and families.

The role emphasizes collaboration with hospital and physician relationships, adherence to HIPAA, and a strong customer-focused approach to improve the patient experience across Diversicare centers.

Qualifikationen

• Must hold an active nursing license (RN preferred).
• 1–2 years’ experience in skilled or long-term care is desired.
• 1–2 years’ sales, marketing or admissions experience preferred.

• Conduct clinical assessments and determine appropriateness with care team for safe transitions.
• Collaborate with center teams to meet admissions goals.
• Present Center features/benefits using Diversicare Sales Steps.
• Maintain clear written and verbal communications.
• Advocate for quality of care and customer experience.
