Job summary

Elevate seeks a Transition Specialist to provide primary and secondary crisis services for children and families, meeting clients in homes, schools, or other settings. You will coordinate care with health networks and external partners, manage a caseload, and document progress in the Electronic Health Record.

You will engage families, promote program referrals, and ensure timely follow-up while collaborating with supervisors and the Elevate Team to support recovery and positive outcomes.

Qualifications

  • Bachelor’s degree in human services or related field.
  • 2-3 years' experience working with children and families in community-based programs.
  • A valid Iowa Driver's license and dependable transportation.
  • Crisis Intervention with experience in suicide intervention, and postvention.
  • Knowledge of community resources.
  • Experience assisting people with addiction, relationship difficulties, depression, stress, extreme poverty, mental illness, parenting etc.

Responsibilities

  • Complete outreach to promote programs within the 14-county program providing support and advocacy for juveniles.
  • Ensure contract deliverables are met and grant requirements are adhered to.
  • Receive referrals for programs and ensure referrals are followed up on in a timely manner consistent with program requirements.
  • Meet with families in their homes or other preferred location with appropriate regularity determined by program requirements and family needs.
  • Complete required documentation and maintain accurate records within the Electronic Health Record.
  • Maintain a caseload.
  • Assign cases to other employees working cases within the scope of this program while providing supervision, support, and maintenance of all case records within the Electronic Health Record.
  • Collaborate with supervisor and/ or Director of crisis services regarding intakes and case needs.
  • Communicate with referring agency/worker, supervisor, and Elevate Team.
  • Promotes integration of services for patients including behavioral health care and long-term services and supports to enhance the continuity of care.
  • Remain available to clients via phone during working hours with some on call hours assigned through a rotation.
  • Identifies critical incidents and develops crisis plans.
  • Engage with clients that have cancelled or no-show to appointments to get them re connected to services.
  • Coordinate for the clients and needed services/resources with both internal and external service providers.
  • Complete Documentation, Risk assessments, On The Way System documentation process.
  • Trainings are on-going with week training online or on location for Boys Town for the On the Way Home service program.

Position Summary

The Transition Specialist performs general functions as a secondary crisis worker in family/child advocacy working within community contracts. The Transition Specialist will provide primary and secondary crisis services for juveniles and their families, working to identify strengths, goals, while addressing potential barriers to success. The Transition Specialist will connect the family with behavioral and mental health resources to promote positive outcomes. Transition Specialist also, performs function as a primary and secondary crisis services for children and families servicing through the course of the Boys Town service delivery model as well as the JCS Transition to Adulthood service delivery model. Helping people where they are when they need help is foundation of Elevate and our service delivery. The On The Way Home program and Transition to Adulthood Programs will be guild and function of service programs while working with referral success and community partnerships to help children and families be successful and reduce the child from reoffending while in program care. The Transition Specialist is responsible for ensuring appropriate delivery of services and coordination of care within health networks and with external entities to collaborate patient care effectively. The Transition Specialist provides excellent patient driven care and demonstrates superior positive regard toward all clients. The Transition Specialist will meet with clients in their homes, schools or other settings as required. The Transition Specialist will function in a multi-tasking, multiple priorities environment, while maintaining accuracy and attention to detail. The Transition Specialist is responsible for providing services to families and children, while providing timely updates of progress, barriers, and concerns to the referring agency and supervisor.
