The Per Diem Transition Unit Physician Assistant (PA) will reflect the mission, vision, and values of Hackensack Meridian Health (HMH). They will comply with all relevant policies, follow established procedures and protocols, and adhere to the HMH Code of Ethics and Corporate Compliance Program. They will be an integral part of the Care Management / Population Health team. The PA will hold a minimum of a Master's degree, maintain active board certification, and meet employment terms. Reporting to the Supervisor of Care Transitions, the PA will work as part of a multidisciplinary team to provide high-quality, cost-effective patient care within their scope of practice. They should be knowledgeable in acute disease management, possess strong clinical and critical thinking skills, and have excellent communication skills. They must be a team player, supporting team members and collaborating with physicians to improve patient outcomes and satisfaction. They will support discharge planning and projects related to length of stay, working to ensure smooth and safe transitions for patients, reducing readmissions. The position requires availability on 1 of the 6 holidays.
Required: Master's degree from an accredited PA program, Physician Assistant license, certification, and relevant certifications such as BLS, ACLS, DEA, and state-specific certifications. Ability to work independently with strong clinical skills and communication.
Preferred: 2-3 years of acute care experience in a hospital setting, Medicare and Medicaid certification.
