Job summary

Centene is seeking a Transition of Care - RN Care Manager to develop, assess, and facilitate complex care management for members with physical needs. You will create personalized care plans and educate families to support healthier outcomes.

Key requirements include an Accredited Nursing degree or RN license, 2–4 years of related experience, and state or compact licensure. This role offers remote work with preference for KY residency or Eastern/Central time zones.

Qualifications

• RN licensure in state or compact required.
• 2–4 years of related experience.
• Bachelor's degree in Nursing or degree from Accredited School of Nursing.

Responsibilities

• Evaluate the needs of the member and barriers to accessing appropriate care
• Develop ongoing care plans/service plans and collaborate with providers
• Identify problems/barriers to care and provide appropriate care management interventions
• Coordinate between the member and care provider team for person-centered care
• Provide ongoing follow-up and monitoring of member status and care plan goals
• Provide resource support to members for local services based on assessments
• Facilitate care management and collaborate with providers to ensure timely access to care
• Perform telephonic, digital, home, and/or other site outreach to assess member needs
• Collect, document, and maintain all member information and care management activities
• Provide education to members and their families on disease processes and care options

Education
