Job summary

Merrimack Health seeks a Transition of Care Pharmacist to lead medication management across the hospital stay and post-discharge follow-up. You will work with a multidisciplinary team to identify and resolve medication-related problems and support adherence for safer transitions.

The role focuses on reducing preventable readmissions and ensuring safe medication use while coordinating with physicians, nurses and social workers to optimize patient outcomes.

Qualifications

• Doctor of Pharmacy (PharmD) degree from an ACPE-accredited pharmacy program or equivalent.
• Pharmacist licensure in the Commonwealth of Massachusetts.
• At least 3 years of Transition of Care Pharmacy experience.
• Certification in Diabetes Education or enrolled in a Diabetes Education certificate program to be completed with the 1st year of employment.

Responsibilities

• Provide comprehensive medication management and transition-of-care services from hospital admission through discharge and post-discharge follow-up.
• Collaborate with physicians, nurses, case managers and other health professionals to ensure safe, coordinated transitions of care.

Job description

JOB SUMMARY:

The Transition of Care (TOC) Pharmacist is responsible for providing comprehensive medication management and transition-of-care services to patients throughout the continuum from hospital admission through discharge and post-discharge follow-up. Working with the multidisciplinary care team, the pharmacist identifies and resolves medication-related problems, promotes safe and effective medication use, supports medication access and adherence, and helps reduce preventable medication-related complications, emergency department visits, and hospital readmissions. The pharmacist collaborates closely with physicians, advanced practice providers, nurses, case managers, social workers, behavioral health professionals, community pharmacies, outpatient providers, and other healthcare team members to ensure a safe and coordinated transition from the hospital to the next level of care.

EDUCATION AND RELATED EXPERIENCE:

Required:

• Doctor of Pharmacy (PharmD) degree from an ACPE-accredited pharmacy program or equivalent.


• Pharmacist licensure in the Commonwealth of Massachusetts.


• At least 3 years of Transition of Care Pharmacy experience


• Certification in Diabetes Education or enrolled in a Diabetes Education certificate program to be completed with the 1st year of employment.

Preferred:

• Certified to practice Collaborative Drug Therapy Management (CDTM) in state of Massachusetts


• Certification in Medication Therapy Management (MTM) or enrolled in a MTM certificate program to be completed within the 1st year of employment


• PGY1 or PGY2 pharmacy residency, preferably in acute care, ambulatory care, medication-use safety, or transitions of care.
