Job description
Overview

Join Southwest Network Team as a Transition Facilitator for teens and young adults (ages 16.5 to 22) transitioning from the children’s behavioral health system to adulthood or the adult system of care. You will coach, support, and help youth with emotional, behavioral, or mental health issues, guiding them in employment, education, housing, social and family relationships, independent living, finances and budgeting, decision making, health and wellbeing, community life, and more using the Transition to Independence Process (TIP).

Responsibilities
  • Engage as a member of a transition team to develop solution-focused strategies to overcome barriers.
  • Evaluate youth to determine suitability to enter the program, then facilitate service plans and progress youth through the program to completion.
  • Collaborate with other service providers and educators from children’s mental health, education, adult mental health, child welfare, rehabilitation and other partners to ensure appropriate support for each young adult.
  • Work in the community and during non-traditional hours, at times and places that work best for the young adults.
  • Perform administrative and documentation functions related to the role.
Qualifications
  • Must possess one of the following education/experience combinations:
  • Master’s or bachelor’s degree with a behavioral health or related field;
  • Bachelor’s degree in a non-related field plus one year related experience;
  • Associate’s degree in any field plus one year related experience;
  • High school diploma or equivalency certificate plus three years’ related experience.
  • At least one year of behavioral health experience facilitating Child and Family teams (preferred).
  • Experience with transition-aged youth in behavioral health, foster care and/or juvenile justice systems (preferred).
  • System experience and understanding of service planning for seriously mentally ill adults (desired).
  • Must maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network).
  • Must maintain a valid Arizona state fingerprint clearance card.
  • Must maintain a valid Arizona driver’s license and car insurance.
  • Must be at least 21 years of age.
