Job summary

Infopact, Inc. is seeking a Transition Counselor to provide individualized counseling and lifecycle case management for transitioning Soldiers and eligible clients.

The role covers initial counseling through Capstone validation, helping clients craft practical transition plans and connect to resources. You will deliver one-on-one counseling, group sessions, and virtual services using government-approved curriculum, maintaining ongoing engagement and ensuring required documentation is completed

Qualifications

  • Bachelor’s degree required.
  • At least two years counseling/advising experience.
  • Knowledge of counseling principles, case management, adult learning, and military-to-civilian transition challenges.
  • Ability to facilitate groups and provide effective in-person/virtual instruction.
  • Strong interpersonal, documentation, time-management, and customer service skills.
  • Ability to manage a caseload, meet deadlines, protect PII, and maintain accurate records.
  • Ability to obtain required Government suitability and system access.

Responsibilities

  • Help clients develop and execute Individual Transition Plans.
  • Deliver one-on-one counseling, group instruction, and briefings.
  • Maintain continuous case engagement through Capstone and transition.
  • Coordinate referrals to VA, DoL, education providers, and partner agencies.
  • Document all services and actions in the designated system of record.
  • Support client intake, logistics, attendance, and records management.
  • Participate in quality reviews, training, reporting, and outreach.

Infopact Inc, founded in 2004, is an SBA-certified 8(a) Woman-Owned Small Business specializing in providing technical and management operations support for Department of Defense and Federal civilian agencies. We are currently looking for a Transition Counselor to assist our client.

The budgeted compensation for these positions includes an hourly rate of $29.57 plus additional health and welfare payment of $5.09/hr. Hourly rates are variable based on geographical locations as this is an SCA contract.

Position Summary

Provides individualized counseling, group instruction, and lifecycle case management totransitioningSoldiers and eligible clients. The Transition Counselor serves as the primary case manager frominitialcounseling through Capstone validation, helping each client complete required activities, develop a practical transition plan, meet Career Readiness Standards, and connect to appropriate resources.

Key Responsibilities
  • Conduct initial counseling, assess transition readiness, interpret assessment results, and assign theappropriate tierlevelin accordance withArmy TAP requirements.
  • Help clients develop, update, and execute Individual Transition Plans aligned to employment, education, entrepreneurship, vocational, and personal goals.
  • Deliver one-on-one counseling, group instruction, briefings, workshops, and virtual services using Government-approved curriculum and materials.
  • Maintain continuous case engagement andrequiredcontact cadence from enrollment through Capstone and transition completion.
  • Guideclients in completing Career Readiness Standards andrequireddocumentation, including DD Form 2648 and associated records.
  • Schedule and reschedule appointments, follow up on missed services,identifybarriers, and coordinateappropriate interventions.
  • Document all services, milestones, assessments, referrals, warm handovers, and follow-on actions accurately and on time in the designated system of record.
  • Coordinate warm handovers to the Department of Veterans Affairs, Department of Labor, education providers, employment resources, community partners, and other agencies asappropriate.
  • Support client intake, registration, classroomlogistics, attendance, records management, surveys, and other administrative functions needed for compliant delivery.
  • Participate in quality reviews, training, reporting, outreach, mobile support, and surge coverage as assigned.
Minimum Qualifications
  • Bachelor’s degree from an accredited institution.
  • At least two years of experience counseling or advising individualsregardingbenefits, employment, education, career planning, military transition, or community resources.
  • Knowledge of counseling principles, case management, adult learning, career development, and military-to-civilian transition challenges.
  • Ability tofacilitategroups and provide effective in-person and virtual instruction.
  • Strong interpersonal, documentation,time-management, computer, and customer service skills.
  • Ability to managea caseload, meet deadlines, protect personally identifiable information, andmaintainaccuraterecords.
  • Ability to obtain andmaintainrequired Government suitability, installation access, Common Access Card, and system access.
Preferred Qualifications
  • Priormilitary service, military spouse experience, or direct experience supporting Service members, veterans, and families.
  • Direct knowledge of and experience with Army TAP curriculum, Career Readiness Standards, Capstone,SkillBridge/Career Skills Programs, and interagency transition resources.
  • Professional certification in career counseling, workforce development, coaching, human resources, ora relateddiscipline.
