Job summary

Beauregard Home Health, part of the UnitedHealth Group family, is seeking Physical Therapists for Full Time, Part Time or PRN positions. The role involves evaluating patients, planning and delivering PT care in home settings, and coordinating with the care team to improve mobility and function.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the plan of care.
• Provides services within the scope of practice per state laws and plan of care, in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs health care team, patient, and family regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment; completes and submits progress notes per policy.
• Completes all evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
