Job summary

Beauregard Home Health, a part of LHC Group, part of UnitedHealth Group, seeks a Physical Therapist to assess and treat patients in their homes. You will develop plans of care, perform interventions to improve mobility, and monitor progress with a focus on functional status and pain management.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance or access to dependable transportation.
• Ability to function in diverse home environments and coordinate with care team.

Responsibilities

• Evaluates patient functional status and needs; assists physician in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to nurse/physician.
• Treats patients to relieve pain, restore function, and maintain max functioning.
• Informs patient and caregivers about plan of care and progress toward goals.
• Visits according to Plan of Treatment; submits progress notes per policy.
• Completes PT evaluations and develops plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
