You will collaborate with supervising therapists, review patient histories, and help implement treatment programs that improve mobility and function.

The role requires active state licensure and strong communication with patients, families, and care teams, while maintaining meticulous records and adhering to regulatory standards.

Responsibilities

• Consult with patients to assess condition and review medical history.
• Diagnose movement dysfunction by observing patients and listening to them.
• Develop individualized treatment plans and exercise programs.
• Coach patients and teach therapeutic massage and exercises to ease pain.
• Maintain patient records and track goals in the clinical chart.
• Use standardized assessments and evidence-based practice in interventions.
• Assess progress and adjust treatments per regulatory and clinic requirements.
• Evaluate outcomes and refer as appropriate based on results.
• Provide counseling to patients and families about physical disorders.
• Communicate progress with supervisor and health team members.
• Prepare comprehensive discharge summaries per regulatory requirements.
• Document all regulatory information in electronic systems.
• Participate in trainings, in-services and conferences.
• Able to supervise rehabilitation aides and students.
• Adhere to company policies and code of conduct.
