Job summary

PAM Health seeks a Transfer Coordination LVN to triage and coordinate patient transfers from other facilities. You will monitor triage needs, accept referrals, and communicate promptly with referral sources, physicians, and nurses to ensure smooth transitions.

Preferred applicants have a behavioral health or substance use background, strong customer service, and excellent verbal and written communication skills, with solid organizational abilities.

Qualifications

• Valid LVN license required; CPI and CPR within 30 days.
• Behavioral health/substance use experience preferred.
• Excellent customer service and communication; organized and multitask effectively.
