Adventist HealthCare's Shady Grove Medical Center in Rockville, MD is seeking a Transfer Center Nurse Coordinator for the night shift (7 PM–7:30 AM, Mon-Fri). You will coordinate interfacility transfers and provide nursing care, educating patients and families through the continuum of care, while mentoring Team I nurses and upholding ANA standards.

Qualifications include a Maryland RN license, BLS, and 1 year of RN experience; ACLS/PALS/NRP as required by specialty area.

Qualifications

• Minimum 1 year of RN experience.
• Maryland RN license and BLS required; ACLS/PALS/NRP as required by specialty area.

Responsibilities

• Educate patients and families through the continuum of care.
• Serve as role models to Level I nurses.

• Develop a plan of care according to patient population.
• Identify impending changes in patient condition and initiate corrective action.
• Mentor students, colleagues, and team members in patient-centered care.
• Conduct purposeful rounding from patient and family perspective.
• Conduct literature searches and apply findings to practice.
• Identify opportunities for improvement in clinical care and participate in change of practice based on evidence.
• Facilitate interfacility patient transfers and enter orders.

About This Role

The Transfer Center Nurse Coordinator is responsible for coordinating patient transfers and providing nursing care during the night shift. This role is ideal for experienced nurses who are dedicated to patient-centered care and professional development.

Requirements

Education:

• ACLS, PALS, NRP as required by specialty area (not needed for transfer unit)
