Job summary

The Transfer Management Nurse at Henry Ford Health in Detroit will coordinate patient transfers into, out of, and between hospitals, aligning providers, care teams, medical transportation, and financial clearance to ensure safe, timely transfers.

You will bring a Bachelor's degree in nursing, Michigan RN licensure, and at least three years of clinical or capacity-management experience to prioritize transfer issues, maintain discretion with confidential matters, and support system-wide patient

Qualifications

• Bachelor's degree in nursing or equivalent.
• Three (3) years of clinical experience or capacity management experience.
• Michigan RN licensure required; experience with patient flow and transfer processes preferred.

Responsibilities

• Coordinate patient transfers with providers, care teams, and medical transportation.
• Ensure transfers to appropriate level of care and complete necessary steps for care progression.
• Collaborate with financial clearance teams to streamline transfer processes.
• Prioritize and resolve transfer issues using clinical judgment and discretion.
• Maintain confidentiality and exercise independent judgment in sensitive matters.
