Qualified candidates will have a BSN and a minimum of three years of nursing experience in acute care.
Job Description - TRANS CARE NURSE NAVIG Inpatient- POOL - FCCC (252198)
In collaboration with the patient/family, physicians, and the interdisciplinary team, the Transitional Care Nurse Navigator facilitates the patient's progress through the acute episode of care in an efficient and cost-effective manner. The role includes creating a transition plan with the patient, family, and providers for moving from inpatient to post-acute settings. The nurse initiates evidence-based protocols and order sets based on the patient's admission diagnosis, influencing care progression and proposing an appropriate transition plan within 48 hours of admission. The Navigator advocates for the patient and family throughout the care continuum and facilitates communication among the multidisciplinary team, referring physicians, and ambulatory care team. They also foster patient/family self-management and ensure compliance with the care plan via post-acute follow-up calls.
