Job summary

PruittHealth Home Health is seeking a Physical Therapist to provide evaluative, consultative, and treatment interventions for patients in their homes. The role offers a high base pay with a $15,000 sign-on bonus and $5,000 relocation assistance for qualified candidates, with special consideration for those in Vidalia & Dodge County, GA.

As part of our Home Health team, you will conduct visits, collaborate with family and an interdisciplinary team, and ensure accurate documentation and ongoing

Qualifications

• Must hold a valid professional license in the state of practice.
• Graduate of an accredited Physical Therapy Program.
• If foreign-trained, the program must be deemed substantially equivalent by a credentials evaluation organization approved by the APTA.

Responsibilities

• Evaluate the patient's functional level and skilled therapy needs.
• Treat the patient to relieve pain, restore function, and maximize outcomes per the care plan.
• Advise and consult families and the inter-disciplinary team to promote outcomes.
• Document progress using required forms and timelines (OASIS, SOC, admissions, recertifications, ABNs).
• Maintain safety standards and coordinate with the physician to manage the care plan.
