Job summary

PruittHealth Home Health is seeking a licensed Physical Therapist to deliver evaluative, consultative and therapeutic interventions in patients' homes in Craven County, NC. The role supports a part-time or PRN schedule with a focus on safety, quality, and patient-centered outcomes.

The PT will assess functional status, develop and implement a plan of care, and collaborate with families and the home health interdisciplinary team to ensure timely, appropriate care. Licensure in-state required.

Qualifications

• Must hold a valid PT license in the state of practice.
• Graduate of an accredited Physical Therapy Program.
• Foreign-trained graduates must have credentials evaluated for equivalence by a recognized body.

Responsibilities

• Evaluate patient function and skilled therapy needs.
• Treat to relieve pain, restore function, and achieve care plan goals.
• Advise families and the interdisciplinary team to optimize outcomes.
• Document progress using required forms and timelines.
• Collaborate with the team to update the plan of care and coordinate referrals.
• Maintain clinical records and ensure regulatory compliance.
