GENERAL DESCRIPTION

Performs complex (journey-level) social service or case management work involving the care and community reintegration of youth who have been adjudicated delinquent of felony offenses, committed to the agency by a juvenile court, and placed directly in a low to medium security halfway house or placed in a halfway house as a gradual transition from a TJJD high-security correctional facility back into their home community.

Provides living skills training and coordinates youth participation in activities relating to community reintegration in collaboration with halfway house administrative and direct-care staff, TJJD case managers, and TJJD parole officers.Interaction with representatives from community organizations and groups.

Coordinates health care activities to facilitate youth access to medical and psychiatric services and to help ensure the medical and medication needs of youth are met while assigned to the halfway house and the continuity of applicable medical care upon release.Collaborates with the halfway house case managers and with the University of Texas Medical Branch Correctional Managed Care (UTMB/CMC) nursing and health care staff contracted to provide medical, dental, psychiatry, pharmacy, and nursing services for youth in TJJD operated facilities, including onsite care provided in-person and through Digital Medical Services (DMS) sessions.May be assigned to assist with supervision of daily activities as needed at the halfway house.May train other staff.

Works under general supervision with moderate latitude for the use of initiative and independent judgment.May be required to work in excess of 40 hours per work week and be on-call.

ESSENTIAL FUNCTIONS

Recruits new community service sites for youth to perform community service. Maintains relationships with current community sites and acts as a liaison between the sites and the halfway house.

Acts as a community liaison for various actions relating to reintegration, which include obtaining governmental records such as birth certificates, Texas IDs, social security cards; enrolling youth in public schools and coordinating the withdrawal process upon release into the youth’s home community; coordinating participation in high school diploma or equivalent preparation classes and registration for testing; and assisting youth in gaining employment and with understanding work and pay schedules.

Prepares Independent Living Training Modules and teaches Independent Living Skills to youth.

Coordinates weekly and monthly community service and recreational activities.

Completes weekly recreational activity reports and data entry and reviews monthly reports regarding completed community service hours to ensure accuracy.

Provides intake health screening and health services orientation to youth upon arrival to the halfway house based on review of the youth’s medical/medication files; notifies appropriate staff of any medical, health-related, or dietary matters; completes appropriate documentation.

Receives, responds, and documents youth sick call requests; implements Health Care Protocols for the Care of Minor Health Conditions under the direction of the agency’s Medical Director; administers medication to youth; coordinates medical appointments for youth per agency and UTMB policy; meets with nursing staff to review past and present medical concerns.

Coordinates appointments and medication needs upon release, including the Texas Department of State Health Services (DSHS) appointments and medication needs; communicates medical needs to parents and guardians as needed.

Provides initial and ongoing guidance and instruction to direct-care staff as necessary, including instruction and training regarding sick call requests, medication distribution, use of the pharmacy replacement system (PRS system), and completion of various health care forms.

Collaborates with medical staff in meeting established objectives and goals; shares information, identifies concerns, and recommends solutions or improvements to resolve problems or enhance operations.

Prepares and maintains First Aid kits and medical supplies; maintains medical/medication files and documentation; maintains control and documentation of prescription medication.

Coordinates the transportation of youth locally to community service sites, employment sites, education locations, locally and long distance for medical appointments, due process hearings, and for other reasons.

Assists nursing staff in scheduling Digital Medical Services (DMS) sessions through which youth are provided access to psychiatric and medical providers; collects information for DMS providers; monitors the DMS sessions; acts as liaison between the DMS providers and case managers, and other halfway house as needed, to communicate relevant information and observations.

Performs a variety of related duties not listed, to be determined and assigned as needed.

Performs all duties in compliance with agency safety policies and procedures:

  • Reports safety hazards and corrects hazards when possible.
  • Completes required documentation in the event of an accident/injury within requested time frames.

Attends work on a regular and predictable schedule in accordance with agency attendance and leave policies.

Qualifications

MINIMUM QUALIFICATIONS

Bachelor’s degree with major course work in health care, behavioral science, social science, criminal justice-corrections (not law enforcement or police sciences), education, or related field. One year of full-time wage earning experience that may be any combination of the following: coordinating community resources, case management, or medical services, which may include services for at risk or disadvantaged populations; experience in an adult or juvenile correctional, mental health, or detention setting; or experience in the health care field or involving review of medical records.

OR

High School diploma or equivalent. Five years of full-time wage earning experience that may be any combination of the following: coordinating community resources, case management, or medical services, which may include services for at risk or disadvantaged populations; experience in an adult or juvenile correctional, mental health, or detention setting; or experience in the health care field or involving review of medical records.

Required:

  • Must have and maintain a valid driver license with an acceptable driving record.

Preferred Experience:

  • Education or experience in the health care field or experience with medical records.

Experience and Education Substitutions:

  • One year of appropriate experience may substitute for one year of college (30 course hours) on a year for year basis.
  • Completed course hours of undergraduate study may substitute for up to four years of experience on a basis of 30 course hours for one year of experience.
  • Completed course hours of graduate study may substitute for up to two years of experience on a basis of 12 graduate course hours for one year of experience.
  • Pro-rated part-time experience may satisfy the experience requirement.

Note: Any degree, diploma, high school equivalent, or course hours required to meet minimum qualifications must be from an accredited educational institution, college or university.
