About Advanced Thyroid & Hormone Clinic

Advanced Thyroid & Hormone Clinic, founded by Dr. Amie Hornaman, The Thyroid Fixer, specializes in individualized thyroid optimization, bioidentical hormone replacement therapy (BHRT), and functional medicine.

Our mission is to provide patients with the personalized attention, comprehensive laboratory interpretation, and thoughtful clinical guidance that is often missing from conventional healthcare.

We believe thyroid and hormone care should extend beyond a single lab value or standardized protocol. Our approach centers on understanding each patient's unique symptoms, laboratory findings, medical history, and response to treatment.

We are seeking an experienced, compassionate Nurse Practitioner, Physician Assistant or Naturopathic Doctor, who is passionate about functional medicine and enjoys working closely with patients throughout their optimization journey.

This is a fully remote, part‑time opportunity for a clinically knowledgeable practitioner who values flexibility, collaboration, and delivering exceptional patient care.

Position Overview

As a Practitioner with Advanced Thyroid & Hormone Clinic, you will serve as a clinical consultant, providing comprehensive thyroid, hormone, and functional medicine evaluations and ongoing patient management.

You will work collaboratively with our established prescribing partners to develop individualized treatment recommendations, monitor patient progress, and recommend appropriate adjustments throughout each patient's program.

Independent prescribing authority is not required. Our designated prescribing partners are responsible for evaluating and authorizing prescriptions.

Your primary focus will be clinical evaluation, laboratory interpretation, individualized treatment recommendations, patient education, and ongoing monitoring.

This is not a traditional primary care position. Rather, it is a specialized clinical consulting role focused on advanced thyroid, hormone, and functional medicine care.

Primary Responsibilities
  • Conduct telehealth consultations and ongoing patient follow-up appointments.
  • Review and interpret comprehensive thyroid, hormone, and metabolic laboratory panels.
  • Evaluate patient symptoms, health histories, laboratory findings, and responses to current treatments.
  • Develop individualized treatment recommendations in collaboration with established prescribing partners.
  • Recommend thyroid treatment strategies, including T4, T3, combination therapy, and natural desiccated thyroid when clinically appropriate.
  • Evaluate and recommend bioidentical hormone replacement strategies involving estradiol, progesterone, testosterone, and related hormone therapies.
  • Monitor patient response to treatment and recommend medication adjustments when appropriate, in collaboration with prescribing partners.
  • Assess additional functional and integrative medicine therapies when clinically indicated, including low-dose naltrexone (LDN), GLP-1 therapies, peptides, spironolactone, oxandrolone, and other relevant modalities, subject to applicable regulations.
  • Guide patients through the entire duration of their clinical program, providing ongoing education, monitoring, and individualized support.
  • Respond to patient questions and concerns with empathy, professionalism, and timely communication.
  • Collaborate with health coaches, prescribing partners, and other clinical team members to ensure continuity of care.
  • Recognize signs of medication intolerance, contraindications, adverse effects, or concerns requiring additional medical evaluation.
  • Maintain thorough and timely clinical documentation.
  • Participate in clinical case discussions, team communication, and ongoing education.
  • Follow applicable clinical guidelines, professional standards, and scope‑of‑practice requirements.
Required Qualifications
  • Active, unrestricted Nurse Practitioner, PA, ND license.
  • Clinical experience in thyroid management and bioidentical hormone replacement therapy.
  • Strong knowledge of comprehensive thyroid laboratory interpretation, including TSH, Free T3, Free T4, Reverse T3, and thyroid antibodies.
  • Familiarity with individualized thyroid replacement strategies, including T3, T4, and natural desiccated thyroid.
  • Experience with female hormone optimization, particularly perimenopause and menopause.
  • Knowledge of testosterone replacement and related hormone concerns.
  • Strong clinical assessment, critical‑thinking, and problem‑solving abilities.
  • Experience delivering care through telehealth or proficiency with virtual clinical platforms and electronic health records.
  • Exceptional written and verbal communication skills.
  • Ability to manage an ongoing patient caseload with organization, accuracy, and attention to detail.
  • Strong commitment to compassionate, personalized patient care.
  • Ability to work independently while collaborating effectively with prescribing partners and other clinical team members.
Preferred Qualifications
  • Specialized BHRT training through BHRT Training Academy, A4M, or similar functional or integrative medicine education programs.
  • Additional education or certification in functional medicine or integrative medicine.
  • Advanced thyroid‑management training and experience with individualized T3 and T4 treatment strategies.
  • Experience with GLP-1 therapies, low-dose naltrexone, peptides, metabolic medicine, or adjunctive hormone‑related therapies.
  • Previous experience in a cash‑pay, concierge, functional medicine, or specialized telehealth practice a plus.
  • Additional relevant clinical certifications and professional training.

We welcome experienced practitioners who are excited to continue expanding their knowledge and clinical skills.

Candidates should be open to learning our clinic's approach to thyroid and hormone optimization and collaborating closely with our clinical team while maintaining sound, independent clinical judgment.

Professional Requirements
  • Maintain an active, unrestricted license and other applicable professional credentials.
  • Maintain personal medical malpractice/professional liability insurance appropriate for the services performed.
  • Complete all continuing education required to maintain active licensure and professional competency.
  • Remain current with applicable telehealth, clinical documentation, privacy, and professional practice requirements.
  • Practice within applicable licensing and scope‑of‑practice regulations.
  • Collaborate with the clinic's designated prescribing partners regarding all medication recommendations and prescription implementation.

Independent prescribing authority is not required for this position.
