Job summary

Affinity Home Health Care Inc seeks a Therapy Team Leader to blend clinical expertise with team oversight, driving measurable patient outcomes across home health services.

The role supervises therapists, assistants, and technicians, develops therapy plans, monitors treatments, and leads quality initiatives while coordinating schedules, documentation, and staff development.

Qualifications

• Current state licensure as a PT, OT, or SLP.
• Minimum of three years' relevant experience.
• CPR/BLS certification required or obtained within 30 days of hire.
• Active on Therapy Clinical Ladder or holding professional certification preferred.
• Leadership experience is desirable.
