What to Expect (Job Responsibilities):
- Perform telephonic and/or virtual assessments to identify participants' needs
- Provide recommendations to managed care organizations for supportive services required
- Conduct objective assessments for program participation and determine appropriate levels of support
- Educate program participants and MCO representatives on options for home modifications and assistive technology
- Document all member encounters according to established standards
What is Required (Qualifications):
- Graduate of a college-level program in physical therapy, occupational therapy, or an accredited two-year program for a Physical Therapist Assistant or Occupational Therapist Assistant
- Minimum of 2 years’ experience in the social service or healthcare field
- Current active, valid, and unrestricted license or certification as a physical therapist assistant or occupational therapist assistant in the applicable state
- Bilingual Vietnamese required
