What to Expect (Job Responsibilities):
- Perform telephonic and/or virtual assessments to identify participants' needs
- Provide recommendations to managed care organizations for supportive services required
- Conduct objective assessments for program participation to determine appropriate levels of support
- Educate program participants and MCO representatives on home modifications and assistive technology options
- Document all member encounters per documentation standards
What is Required (Qualifications):
- Graduate of a college-level program in physical therapy, occupational therapy, or an accredited two-year program for a Physical Therapist Assistant or Occupational Therapist Assistant
- Minimum of 2 years of experience in social services or the healthcare field
- Current active, valid, and unrestricted license or certification as a physical therapist assistant or occupational therapist assistant in the applicable state
- Certification and/or licensure appropriate to the field of specialty as required
